Deptford Housing Co-op Incident Report Form
An ‘incident’ is something that is an undesired event that has caused harm, damage, concern or distress which happens within Deptford Housing Cooperative or in close proximity to it.
This could be to an individual (staff member, MC member, DHC member, contractor or member of the public), to property, to the reputation of the organisation, or a financial issue.
Any time an incident occurs it is important that we record it so that any necessary actions are taken, and that we learn from it to reduce risk in the future.
- Please complete this form at the earliest opportunity after the incident.
- Where possible stick to known facts – please write as much detail as you remember, expanding each box if required.
- This document will be forwarded directly to the Housing Manager but please keep a copy for your records. You may save a draft that will be sent to your email address.
- If the incident involves a member of the MC or staff member you may prefer to send the report to one of the DHC Officers, either Chair, Secretary or Treasurer.
